Billing & Insurance
We accept most local and many national insurance plans. However it is your responsibility to understand whether your insurance provider limits or otherwise dictates the doctors you can see or the services you can receive under your coverage.
If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier for payment. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. If your procedure is not covered by insurance, you will be directly billed for the full amount. Please contact our billing office or call your insurance carrier should you have questions.
Please bring your insurance card to every appointment. You must notify our office of any secondary insurance policies you may have.

Insurance Participation
We participate with many insurance companies; however, it is your responsibility to check with your insurance company to ensure that we participate. If your insurance company requires a referral from your primary care physician, please be sure to obtain it prior to your visit. Remember to bring your referral authorization and your health insurance card to your visit.
